What does our Health and Wellness Program do?

Our Health & Wellness Program is designed to support overall health through personalized, practical, and sustainable lifestyle guidance. We work with individuals at all stages of their health journey, whether they are managing a chronic condition, building healthier habits, or simply looking for support to feel better and live well. The program is open to all community members, and no referral is required to participate. Individuals may self-schedule at any time, though referrals from healthcare providers are always welcome and help support coordinated care.

Services offered through the program include nutritional counseling and education, customized exercise programming, one-on-one personal training as availability allows, smoking and tobacco cessation support, general health and wellness counseling, and ongoing lifestyle education and support. All services are individualized to align with each person’s goals, health history, and current needs, with an emphasis on long-term, sustainable behavior change rather than short-term fixes.

While referrals are not required, many participants are referred to the Health & Wellness Program by providers who recognize the value of additional lifestyle support. Common referral sources include primary care providers, physical therapists, chiropractors, behavioral health providers, and medical specialists. Individuals are often referred for support with weight management, nutrition education, chronic condition management, improving strength, mobility, or physical activity tolerance, transitioning out of physical therapy, smoking or tobacco cessation, stress management, or general wellness and health behavior change.

The Health & Wellness Program is designed to complement the care individuals are already receiving. When referrals are involved, the Health & Wellness Coach works closely with the healthcare team to ensure a coordinated, supportive, and patient-centered approach. Whether someone self-schedules or is referred, the goal remains the same: to provide education, guidance, and support that empowers individuals to build healthier habits, improve quality of life, and maintain lasting health improvements.
